Q. Does your facility offer "respite" services?
A. Yes, we are able to offer respite services depending on bed availability. If you are interested in respite
services contact
Lisa Hubbard of Resident and Family Services with projected dates that this service will be
needed as soon as possible. Respite services can be billed to Medicaid with prior approval or will be billed
as private pay.
Q. What is the procedure to apply for admission?
A.  If you or your loved one is hospitalized at the time of application you will be approached by the
hospital discharge planner or social worker regarding your options for placement. When you have identified
the facilities you are interested in the hospital will fax information regarding your condition and need for
care. These documents are called a Patient Review Instrument (PRI) and a screen. The facility may request
more information as needed. If appropriate, a bed offer will be made and arrangements made for
admission. If you or your loved one is applying from home you will need to contact an agency to complete
the Patient Review Instrument and screen. In Fulton-Montgomery counties the community health center
will send a certified nurse to the home to complete this document. Most other counties rely on their county
nurses for this service. Once the document is complete it should be delivered to the facility you are
interested in either via fax or mail. At that point the information is reviewed and determination made if a
bed can be offered.
Please Note the facility social worker will also be asking financial questions at this
point. You may be requested to complete an application outlining your insurance coverage and other
resources.
Q. What is the PRI (Patient Review Instrument) and screen?
A.  The PRI is a four page document that looks at the medical condition and need for assistance for the
applicant. This document is "scored" based on the entries. The "score" given determines the level of care
(skilled nursing, assisted living, adult home or home with services) that is most appropriate. The screen is
also used to assess placement focusing on availability of help in the home and potential for discharge.
Please note community agencies do charge a fee for completing these documents.
Q.  What should I bring on the day of admission?
A. On the day of admission we encourage you to bring any personal items that are important to you or
your loved one: photographs, plants, etc.. Basically anything that will make the room feel comfortable and
familiar. We have found that it is good to have 5-7 outfits of clothing. Everyone does get dressed on a
daily basis. Please bring whatever you would normally wear at the home. Each room has a phone jack. If
you would like  a private line in the room we will assist you in contacting the local telephone company. You
will need to bring your own telephone. The facility provides cable service but we do ask Residents to bring
their own television (up to 30 inches). We also request that you bring your insurance cards and social
security card so that copies can be made. If there is a power of attorney we would also like a copy of that
paperwork. Any advance directives such as health care proxy living will or DNR should also be supplied so
that they can be attached to the Residents chart.
